Before we get started, I ask all new clients to complete a short onboarding process. These forms allow me to learn about your health history, fitness background, and personal goals so I can design a program that’s built specifically for you.
Please complete all forms honestly and thoroughly — the more I know about you, the better I can serve you.
Your information is completely confidential and will never be shared with anyone.

 

What’s Included:
•  Client Intake Form — Tell me about yourself and your fitness background
•  PAR-Q Health Screening — A quick health and safety questionnaire
•  Liability Waiver — Required before your first session
•  Goal Setting Form — Help me understand exactly what you want to achieve
